New Patients

The Richmond Road Medical Centre (RRMC) is now accepting new patients.

We encourage all our patients to enroll with us as their preferred provider of general practice services. This enables us to best support your health needs, allows the practice to claim any Ministry of Health subsidies you may be entitled to, including access to cheaper visits, subsidised programmes and in some cases payment for assessments.

Patients can only be enrolled at one practice at any one time. However once enrolled, patients are free to attend any practice or emergency centre as required without limitations.

If you have not been seen in the practice for three years your enrollment becomes invalid and your funding stops. We will however inform you should this occur.

New Patient Enrollment

The Ministry of Health requires a signature to confirm your enrollment. This can be done by a parent or guardian for those under 16 years old. Patients may be asked for proof of identity at the time of enrollment

Completing enrollment prior to your first visit provides us with the opportunity to request your medical records and in most cases review these before seeing you.

Please print and complete the forms as per the steps below and return these to the practice.

Scanned or photographed forms sent electronically and paper forms returned to the practice, are accepted.

  • Step One: Enrollment Form

Print enrollment form, complete and sign.

A separate form is required for each person enrolling. If you were not born in New Zealand, please send or bring a copy of your Passport and Visa.

  • Step Two: Medical History Form

It is important that we know your full medical history, so please complete this Medical History Form.

  • Step Three: Patient Transfer Form

If you are moving from another general practice in New Zealand, please complete this Transfer Form. Completion of this form enables us to request your medical records from your previous doctor and for the practice to access any Ministry of Health subsidies you are entitled to when you visit.

  • Step Four: Access Patient Portal

We encourage all patients over 16 years to register with the Patient Portal. This enables you to make appointments and order repeat prescriptions online. It also gives you access to your test results, immunisation records, recalls, medical warnings and long-term medications. Enrollment into the portal is optional and can be done at any time.   • Step Five:  Deliver the Forms

Please return the forms to the RRMC at 452 Richmond Rd, Grey Lynn or email it to
